ABSTRACT

Recently, more and more clinical and basic studies have shown that early intervention of scars after wound healing can shorten the immature period of scars, improve the final outcome of scars, effectively prevent and control the occurrence and development of pathological scars. However, there are many methods for early management of scars with varying quality, and there is a lack of expert consensus on early scar management. Based on the existing clinical evidence of each intervention method and clinical experience of experts, more than 40 domestic experts from Standing Committee of Chinese Association of Plastics and Aesthetics Scar Medicine Branch have reached a consensus on definition, therapeutic principles, therapeutic methods, and other aspects of early scars after multiple discussions and revisions, providing reference for clinical treatment.

ABSTRACT

In 2018, the American Heart Association/American Stroke Association (AHA/ASA) has developed the latest 2018 guidelines for the early management of patients with acute ischemic stroke (AIS), based on the latest evidences. The 2018 guidelines including recommendations on pre-hospital and in-hospital management treatment, has revised and add new recommendations from 2013 guideline. The major changes in 2018 guideline involve applications of brain imaging in early stage, intravenous thrombolysis and mechanical thrombectomy, et al. This review interprets the 2018 guidelines for clinicians to improve the clinical diagnosis, treatment and outcome of patients with AIS.

ABSTRACT

Frequent earthquake,debris flow,mine disaster,landslides,explosions,car accident and other injuries often lead to the occurrence of crush syndrome.Its clinical manifestations include muscle injury in the injured area,usually combined with distant organ dysfunction,such as liver,kidney,heart,lung,gastro-intestinal tract,and even lead to hypovolemic shock,compartment syndrome,rhabdomyolysis,traumatic multiple organ failure.Due to the hidden pathological changes of secondary organ dysfunction,rapid progress, more dangerous prognosis,it can even lead to death.Therefore,the early identification,diagnosis and treat-ment of crush syndrome can significantly improve the prognosis,avoid complications and reduce mortality and disability rate.

ABSTRACT

Post-infectious bronchiolitis obliterans is a severe form of chronic obstructive lung disease in children that follows an insult to the lower respiratory tract infection. It′s associated with viruses,bacteria and mycoplasma pneumoniae infection. Persist wheezing,recurrent exacerbation and exercise intolerance are the main manifestations. Mosaic attenuation and air trapping can be seen in HRCT. Lung function test shows obstructive dysfunction. Its prognosis is bad. Early identification and management of severe post-infectious bronchiolitis obliterans can improve prognosis greatly.

ABSTRACT

Objective To summarize the experience during early stage of postoperative monitoring and treatment for the patients follwing lung transplantation in surgical intensive care unit,and to improve the patient's outcome of lung transplantation.Methods A retrospective analysis of 9 cases of orthotopic lung transplantation admitted to surgical intensive care unit (SICU) of Beijing Anzhen Hospital from May 2005 to May 2011 was made.Results There 5 male patients and 4 female patients with the age 21 -67 (46.1 ± 14.1 ) years old in present group,and among them 6 patients survived (survival rate:66.7% ).The etiological indication of admission were idiopathic pulmonary fibrosis (n =4),pulmonary lymphangioleiomyomatosis ( n =3 ),and bronehiectasis ( n =2).Bilateral sequential lung transplantation ( n =5 ),single left lung transplantation ( n =1 ) and single rigbt lung transplantation ( n =3) were performed.The postoperative duration of mechanical ventilation,stay duration in SICU and in hospital were 2 -32 d,3 -42 d,and 3 -60 d respeetively.Postoperative complication taking place after surgery were acute rejection in 2 cases and pneumonia 4 cases (3 cases were treated with extracorporeal membrane oxygenator).Postoperative in hospital death was 3 cases of 9 resulting from severe intection (n =2) and cireulation failure ( n =1 ).Conclusion Well-planned postoperative management stroategy in SICU plays an important role in outcome control after lung transplantation,including monitor and respiratory and cardiacvascular management,reasonable implementation of vasoactive medications and diuretics,protective mechanical ventilation strategy,reducing the time of ventilation.

ABSTRACT

[Objective]On May 12,2008,an over 8.0-magnitude earthquake occurred in Wenchuan county,Sichuan province,China.The anthor's field hospital was deployed to An county,which arrived on the 3rd day after the disaster.During the first week of the deployment,284 victims with musculoskeletal injuries received orthopedic management.The objective of this article is to highlight the experience gained by this author's medical assistance team,focusing on the orthopedic care in the early part of the post-disaster relief activities.[Method]Records of 284 patients who underwent orthopedic management were reviewed retrospectively.[Result]There were a large number of musculoskeletal injuries amongst the survivors in the acute phase of the disaster,including spinal trauma,fractures and dislocations,peripheral nerve injuries and soft tissue injuries.The average age was 49.3 years(range,11 to 92 years) with 133 men and 151 women.The patients with upper extremity fracture were 85,with lower extremity fracture 73,with spinal fracture 23,with shoulder or hip dislocation 7.The patients with hand or foot injuries,peripheral nerve injuries and soft tissue injuries were 96.Open injuries were 67,the remaining were closed injuries.Totally 131 patients recovered,and the rest were transferred to the rear hospital for further management.The X-ray postoperatively showed that 21 open fractures were reduced functionally by means of external fixator,and the shoulder or hip dislocations were reduced anatomically.The wounds with primary suture had no infection.There were no other complications.[Conclusion]Correct diagnsis of the injuries,providing scientific and resonable therapeutic measures,and correct operative indication should be emphasized for early orllopedic care.Definitive surgeries such as open reduction and internal fixation are not recommended strongly.External fixation for post-earthquake open fractures is simple and effective,allowing simple approach to wound care and transferring to rear hospital during the whole period of fixation.
